Obstetrics and Gynecology Services (Delivery) with the ERAS Technique - ERACS

Enhanced Recovery After Surgery (ERAS) is a surgical technique developed to speed up treatment and also accelerate patient healing.

In developed countries such as America and Japan, the length of postoperative care is an issue of great concern, they seek to shorten the length of care and recovery of postoperative patients.

They call it ERAS (Enhanced Recovery after Surgery) for surgery in general and in the field of obstetrics they call it ERACS (Enhanced Recovery After Cesarean Surgery) so that the length of stay in the hospital is no more than 24 hours.

Currently in Indonesia this technique has also begun to be carried out, in the Obstetrics and Gynecology Department of the EMC Tangerang Hospital with sufficient modalities a breakthrough has been made to develop a technique called Enhanced Recovery After Cesarean and Gynecology Surgery (ERACGS), with strong interdisciplinary collaboration, both that is Doctor Operators, Anesthesiologists, Pediatricians, Nutritionists and nurses who are carried out during pre-operative, intra-operative and post-operative

The operating doctor plays a role in diagostic enforcement, assesses the level of feasibility of the operation, also determines the level of difficulty that will be encountered in the operating room, the technique for selecting the surgical approach, the modality of the tool to be used, and the width of the incision if an open surgery method is performed.

"The ERAS/ERACS technique makes the incision with a knife with a special sharpness and small size, and with one incision it reaches the fascia, so tissue damage can be minimal." dr. Kondang Usodo, Sp.OG (Obstetrics and Gynecology Specialist at EMC Tangerang Hospital). The incisions are made as minimal as possible, tissue harvesting is also carried out without damaging healthy tissue, avoiding cutting and suturing muscles as much as possible, and when the doctor is sewing the fascia, priority will be given to one-to-one suture techniques. The patient will be given a sweet drink 2 hours before the operation so that it can help to mobilize after the procedure.

The doctor will free adhesions between tissues so that the patient avoids post-healing pain, restores the position of the incised tissue end to end in the same tissue and chooses a thread and suture technique that does not interfere with postoperative movement

The anesthesiologist's role is to carry out a preoperative assessment , choose the most appropriate type of anesthesia and speed up the patient's recovery, and perform anesthesia with an anesthetic needle using a very small pencan model (29). The dose of anesthetic needed for spinal anesthesia is the minimum dose, it can also be combined with regional anesthesia. The anesthesiologist also supervises and provides pain management services while the patient is being operated on until allowed to go home, the infusion and catheter are immediately aff 2 hours after surgery, and the patient has begun to be trained to sit up and walk

The ERACS (Enhanced Recovery After Cesarean Surgery) technique allows the patient to be able to sit while breastfeeding after 2 (two) hours postoperatively, and in the treatment room the patient has been mobilized normally, given drugs and injections through Venflon, eating and drinking as usual and It is hoped that in less than 24 hours the patient will return to activities.

Homecare care support should also be provided for babies and mothers to support the total recovery process

This service is carried out by always adhering to the principle of patient safety. With ERACS , patients will feel comfortable and get a pleasant experience of excellent service
